Motor Vehicle Disabled Parking Permits

Yes. You may submit a disabled parking permit on behalf of another person if: 

• An Application for disabled person parking permit – Form 83039 is completed by the applicant and an acceptable medical provider. 
• A copy of the applicant’s valid Florida identification is provided. 
• An acceptable form of identification from the person submitting the application. 

Acceptable forms of identification include: 

• A valid state-issued driver’s license or identification card. 

or

• A valid passport and proof of legal presence from USCIS. 

Note: An expired U.S. passport, driver’s license or identification card can be accepted if the Special Exception section of Form 83039 is completed by an acceptable medical provider. 

Florida Statute 320.0848 limits disabled parking permits to a maximum of six months, even if a medical provider certifies the disability for longer. After the permit expires, you may apply for one additional six-month permit.
